Hazard: The pacifiers fail to meet federal safety standards. The pacifier mouth guard is too small, posing a choking hazard to infants and toddlers.

Description: This recall involves ‘Bobby Chupete’ pacifiers. The pacifiers have a ring-shaped handle and heart-shaped mouth guard with two ventilation holes. The nipple is made of latex. ‘Bobby Chupete’ and a picture of an infant are printed on the pacifier’s packaging. The pacifier was sold in aqua, red, white or yellow colors.

Sold at: Various retail stores nationwide from November 2004 through July 2009 for about $1.

Maclaren USA Recalls to Repair Strollers Following Fingertip Amputations

Name of Product: Maclaren Strollers

Units: About one million

Distributor: Maclaren USA, Inc., of South Norwalk, Conn.

Hazard: The stroller’s hinge mechanism poses a fingertip amputation and laceration hazard to the child when the consumer is unfolding/opening the stroller.

Incidents/Injuries: The firm has received 15 reports of children placing their finger in the stroller’s hinge mechanism, resulting in 12 reports of fingertip amputations in the United States.

Description: This recall involves all Maclaren single and double umbrella strollers. The word ‘Maclaren’ is printed on the stroller. The affected models included Volo, Triumph, Quest Sport, Quest Mod, Techno XT, TechnoXLR, Twin Triumph, Twin Techno and Easy Traveller.

Sold at: Babies’R'Us, Target and other juvenile product and mass merchandise retailers nationwide from 1999 through November 2009 for between $100 and $360.

Flashlights Sold Exclusively at Target Recalled Due to Burn Hazard

Name of Product: Halloween Flashlights

Units: About 610,000

Hazard: The flashlights can overheat and melt, posing a burn hazard to consumers.

Incidents/Injuries: The firm has received eight reports of flashlights overheating and melting, including one report of burns to the hand.

Description: This recall involves two types of Halloween-themed flashlights: the mini flashlights and flashlights sold with stencils.

Mini Flashlights – The mini flashlights have a key ring extending from the bottom and were sold in a pack of three colors: orange, green and black. The orange and purple packaging has “Mini Flashlights (3 Pack)” printed on the front and “DGI”, “Made in China” and “DPCI# 234-02-1813″ printed on the back.

Standard Size Flashlights – Also included in the recall are standard sized flashlights with a black handle and an orange top. The flashlights were sold with six stencils in various colors and images: a pumpkin, ghost, spider, cat, witch and skull & cross-bones. “Flashlight with Stencil” is printed on the front and “Tien Hsing,” “Made in China” and “234 02 1838″ is printed on the back of the packaging.

Sold exclusively at: Target stores nationwide from August 2009 through September 2009 for $1 for the mini flashlights and $2.50 for the flashlights with stencils.

CPSC Urges Parents and Caregivers to Stop Using “My Baby Soother”

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC) is urging parents and caregivers to immediately stop providing “My Baby Soother” pacifiers to their children. The pacifiers were distributed by T & L Trading Corp., of Brooklyn, N.Y. The pacifiers failed to meet federal safety standards because the nipples can separate from the base easily, posing a choking hazard to infants and toddlers.

T & L Trading Refuses to Cooperate

CPSC is issuing this warning because T & L Trading has refused to recall these pacifiers.

About 16,500 “My Baby Soother” pacifiers were sold at grocery stores, delis, and discount stores in Bronx, Brooklyn, Broadway, and Coney Island of New York from August 2007 through July 2009 for about $1.

The “My Baby Soother” pacifier has a ring-shaped handle and a blue, pink, red, white or yellow heart-shaped mouth guard with two ventilation holes. The nipple is made of either silicon or latex. The pacifier package has the words “My Baby Soother” printed on the top and a picture of an infant on the background.

What Should You Do?

Distributors and retailers who purchased the “My Baby Soother” pacifiers from T & L Trading should immediately stop distributing the pacifier and call CPSC. Consumers should immediately take the recalled pacifiers away from infants and toddlers and discard them.

Clarks Children’s Shoes Recalled Due to Choking Hazard

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Children’s Shoes

Units: About 2,000

Distributor: C & J Clark America Inc., d/b/a the Clarks Companies N.A., of Newton, Mass.

Hazard: Molded rubber pieces on the sole of the recalled shoes can detach, posing a choking hazard to infants and young children.

Incidents/Injuries: No incidents have been reported in the United States. In the United Kingdom there have been six reports of the rubber pieces detaching, including two reports of children mouthing the rubber piece. No injuries have been reported.

Description: This recall involves Clarks children’s shoes sold under the ‘crawlers’ and ‘hazy daze’ style names. The crawlers were sold in infant sizes 2 to 3 1/2; and the hazy daze in sizes 4 to 6 1/2. ‘Clarks’ is printed on the sole underneath the heel and the words ‘Clarks First Shoes’ is printed on a multi-colored label inside the shoe’s heel area. The following names and model numbers are included in this recall.

Recalled due to Strangulation Death of a Child

Name of Products: Horizontal and Vertical Blinds and Cellular Shades

Units: About 15,400 (horizontal), 16,400 (vertical), 800 (Cellular Shades)

Manufacturer: Vertical Land Inc., of Panama City Beach, Fla.

Hazard: Horizontal Blinds: The blinds do not have inner cord stop devices to prevent the accessible inner cords from being pulled out. If an inner cord is pulled out, a child can become entangled in the loop and strangle.

Vertical Blinds: Strangulations can occur if a child’s neck becomes entangled on the free-standing loop formed by the bead chain or by the cord with a weighted device.

Cellular Shades: Strangulations can occur if a child’s neck becomes entangled on the shade’s free-standing looped cord.

Incidents/Injuries: In May 2006, CPSC received a report of a 4-year-old girl from Pensacola, Fla., who strangled in the loop of a vertical blind cord that was not attached to the wall or floor.

Description: This recall involves all Vertical Land custom-made vertical and cellular window coverings that do not have a cord tensioning device and horizontal blinds that do not have inner cord stop devices. A sticker identification label can be found on the head rail of some of the window coverings which lists the company’s name, address and telephone number.

Sold at: Vertical Land stores in Panama City and Pensacola, Fla. from January 1992 through December 2006 for between $60 and $200 (depending on the size of the blind).

Description: This recall involves the Buster Brown & Co. clog children’s shoes with model or retail numbers listed below. These shoes are a plastic molded EVA ‘water’ clog and are designed to resemble a car. The shoes are red, brown, blue, yellow, and pink and most shoes bear the words ‘CARS,’ ‘Transformers Animated,’ or ‘Barbie.’ ‘Buster Brown & Co.™’ is printed on the shoe inside at the heel, on a sewn-in tag inside the fleece-lined shoe, and on the original hang tag. Description: This recall involves the Kolcraft Travelin’ Tot play yards, as well as other similar play yards manufactured for Carter’s, Sesame Street, Jeep, Contours, Care Bare and Eric Carle, with model numbers listed below. Some of the units have a bassinet and/or changing table. Some models have a mobile, vibrating unit or a parent organizer. Model numbers are printed on a white sticker on one of the feet of the play yard.

Hazard: The wooden crib slats can detach or break creating a gap, which can pose an entrapment and strangulation hazard to infants and toddlers.

Incidents/Injuries: LaJobi has received 16 incident reports of crib slat failures, including 10 reports of slats falling out of rails and 6 reports of slats breaking. No injuries have been reported.

Description: This recall involves Bonavita ‘Cabana’ wooden drop side cribs with metal mattress supports. The cribs were manufactured from January 2006 through October 2008 and have production dates between 01 JAN 2006 and 31 OCT 2008. The model name, color, product order (PO) number, production date and other crib identifiers are printed on a label located on the inside bottom rail of the headboard or footboard. The cribs were sold in three finishes, antique white, classic cherry and montego brown. Cribs with other production dates are not affected by this recall.

Recalled Due to Suffocation Hazard

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Eddie Bauer Soothe & Sway Play Yards

Units: About 71,000 in the United States and about 5,600 in Canada

Distributor: Dorel Juvenile Group Inc., of Columbus, Ind.

Hazard: The play yard’s rocking bassinet attachment can tilt even when secured by straps in the non-rocking mode or can stay tilted without returning to a level sleeping surface while in the rocking mode, causing an infant to roll to the corner or side of the bassinet. The infant can then become wedged in the corner or pressed against the side or bottom of the bassinet, posing a risk of suffocation or positional asphyxiation.

Incidents/Injuries: The firm and CPSC have received 10 reports of infants rolling to one side, including 6 that had their faces pressed against the side or the bottom of the bassinet. One child reportedly was turning purple and was out of breath when discovered. No injuries have been reported.

Description: This recall involves Eddie Bauer Soothe & Sway portable play yards. The convertible play yard has a bassinet and changing station feature. A mobile with three teddy bears was also sold with the play yard. Models included in the recall are 05046 (all units) and 05044 units manufactured before December 1, 2008. Different models were sold in Canada. Model numbers and manufacture dates are printed on a sticker on one of the support legs underneath the play yard. Manufacture dates are printed in the YY/MM/DD format.

Sold at: Target, Sears, and Burlington Coat Factory stores nationwide and Internet retailers from January 2008 through May 2009 for about $150.

Recalled Due to Entrapment and Suffocation Hazards

Name of Product: SunKids Convertible Cribs

Units: About 1,900

Importer: Suntech Enterprises Inc., of City of Commerce, Calif.

Hazard: The sides of the crib are made of mesh that expands, creating a gap between the side and the crib’s mattress if it’s not zipped into place, or a gap between the side and an added mattress. A young child can slip into this gap and become entrapped or suffocate. Also, the crib’s drop side can fail to fully latch posing a fall hazard to young children.

Incidents/Injuries: None reported with this crib. However, CPSC is aware of the death of a 5-month-old child in August 2008 involving another company’s nearly identical crib that was recalled. The child became entrapped between the mattress and the mesh side and suffocated.

Description: The recall involves the SunKids convertible crib/playpen/bassinet/bed with model number PY256. ‘SunKids’ is embroidered on the bottom left of the crib’s drop side. The convertible cribs have a drop side rail, stationary side rail, canopy assembly, and bassinet. The sides of the convertible crib are fabric and mesh. The mattress support, bassinet, canopy, and bed skirt are covered in fabric. The fabric and the mesh were sold in navy blue, light blue, pink, beige, white, beige checker, and pink heart.

Product: What’s Inside? Soft Toy Boxes

Units: About 4,000

Manufacturer: Lakeshore Learning Materials, of Carson, Calif.

Hazard: The head of the stuffed butterfly toy can detach from the body, posing a choking hazard to children.

Incidents/Injuries: Lakeshore has received three reports of the head of the butterfly detaching from the body. No injuries were reported.

Description: The recalled product is a soft, multi-colored box with a different color on each side. ‘What’s Inside?’ is embroidered on the front of the box and symbols of a lion, flower and purple butterfly are stitched on another side. Each feely box contains ten small stuffed pieces. The piece affected by this recall is the pink and purple butterfly.

Sold by: Lakeshore Learning Materials stores nationwide and Toys To Grow On™ catalogs from September 2008 through February 2009 for about $40.

Name of product: State Farm Good Neigh Bears

Units: About 800,000 in the United States and 27,000 in Canada

Distributor: State Farm, of Bloomington, Ill.

Hazard: The eyes on these bears can come off, posing a choking hazard to young children.

Incidents/Injuries: State Farm received one report of the plastic eye of a bear coming off and a child placing it in her mouth. No injuries have been reported.

Description: This recall involves the 11-inch (28 cm) and 18-inch (46 cm) State Farm Good Neigh Bears with plastic eyes. The bears are brown and wear a white and red State Farm shirt.

Sold at: The bears were given away free through State Farm agents and at State Farm sponsored events from September 2005 through March 2007.

Hazard: The stuffed toys have two button eyes that could detach from the toy, posing a choking hazard to young children.

Incidents/Injuries: None reported.

Description: The stuffed animal and creature toys are made out of cotton and have plastic button eyes. They come in a variety of styles, such as a rabbit, penguin and reindeer. The recalled toys and style numbers are listed below:

Recalled Due to Fall Hazard

Name of Product: Evenflo Activity Centers

Units: About 213,000

Manufacturer: Evenflo Co. Inc., of Miamisburg, Ohio

Hazard: When used as an activity table, the cap on one end of the product can loosen and fall off, posing a fall hazard to a young child.

Incidents/Injuries: Evenflo has received 11 reports of dislodged end caps which resulted in nine minor injuries, including bumps and bruises. Evenflo has also received a report of a dislodged end cap that resulted in a child suffering a broken collarbone in Canada.

Description: The recall involves Evenflo ExerSaucer Triple Fun stationary activity centers made between October 2006 and December 2008. The model number is 6231711. Stage 3 position involves converting the product into an activity table. No other ExerSaucer models use end caps and, therefore, no other models are included in this recall.

Sold at: Juvenile product and mass merchandise stores nationwide, including Toys ‘R Us and Burlington Baby Depot, from October 2006 through February 2009 for about $120.
