<p><strong>Hazard:</strong> The pacifiers fail to meet federal safety standards. The pacifier mouth guard is too small, posing a choking hazard to infants and toddlers.</p>
<p>Incidents/Injuries: None reported.</p>
<p>Description: This recall involves &#8216;Bobby Chupete&#8217; pacifiers. The pacifiers have a ring-shaped handle and heart-shaped mouth guard with two ventilation holes. The nipple is made of latex. &#8216;Bobby Chupete&#8217; and a picture of an infant are printed on the pacifier&#8217;s packaging. The pacifier was sold in aqua, red, white or yellow colors.</p>
<p>Sold at: Various retail stores nationwide from November 2004 through July 2009 for about $1.</p>

<p>Units: About one million</p>
<p>Distributor: Maclaren USA, Inc., of South Norwalk, Conn.</p>
<p><strong>Hazard:</strong> The stroller&#8217;s hinge mechanism poses a fingertip amputation and laceration hazard to the child when the consumer is unfolding/opening the stroller.</p>
<p>Incidents/Injuries: The firm has received 15 reports of children placing their finger in the stroller&#8217;s hinge mechanism, resulting in 12 reports of fingertip amputations in the United States.</p>
<p>Description: This recall involves all Maclaren single and double umbrella strollers. The word &#8216;Maclaren&#8217; is printed on the stroller. The affected models included Volo, Triumph, Quest Sport, Quest Mod, Techno XT, TechnoXLR, Twin Triumph, Twin Techno and Easy Traveller.</p>
<p><strong>Sold at:</strong> Babies&#8217;R'Us, Target and other juvenile product and mass merchandise retailers nationwide from 1999 through November 2009 for between $100 and $360.</p>

<p><strong>Hazard: </strong>The flashlights can overheat and melt, posing a burn hazard to consumers.</p>
<p>Incidents/Injuries: The firm has received eight reports of flashlights overheating and melting, including one report of burns to the hand.</p>
<p>Description: This recall involves two types of Halloween-themed flashlights: the mini flashlights and flashlights sold with stencils.</p>
<p>Mini Flashlights &#8211; The mini flashlights have a key ring extending from the bottom and were sold in a pack of three colors: orange, green and black. The orange and purple packaging has &#8220;Mini Flashlights (3 Pack)&#8221; printed on the front and &#8220;DGI&#8221;, &#8220;Made in China&#8221; and &#8220;DPCI# 234-02-1813&#8243; printed on the back.</p>

<p>Standard Size Flashlights &#8211; Also included in the recall are standard sized flashlights with a black handle and an orange top. The flashlights were sold with six stencils in various colors and images: a pumpkin, ghost, spider, cat, witch and skull &amp; cross-bones. &#8220;Flashlight with Stencil&#8221; is printed on the front and &#8220;Tien Hsing,&#8221; &#8220;Made in China&#8221; and &#8220;234 02 1838&#8243; is printed on the back of the packaging.</p>
<p>Sold exclusively at: Target stores nationwide from August 2009 through September 2009 for $1 for the mini flashlights and $2.50 for the flashlights with stencils.</p>

<p><strong>Hazard</strong>: The booster seat restraint buckle can open unexpectedly, allowing a child to fall from the chair and be injured.</p>
<p>Incidents/Injuries: Target has received eight reports of the booster seat buckles opening unexpectedly, including three reports of bruises.</p>
<p>Description: This recall involves Circo booster seats with manufacture date codes XJ0811, XJ0812, XJ0901, and XJ0902. The plastic booster seat is blue with green trim and has white straps. The manufacture date code is printed on the backside of the seat next to the consumer warning information. “Circo” can be found on a label located in the front of the seat.</p>
<p>Sold exclusively at: Target stores nationwide from December 2008 through June 2009 for about $13.</p>

<p><strong>Hazard: </strong>The hinges that hold the stair gate in place can break, posing a fall hazard to children if the gate is placed at the top of the stairs.</p>
<p>Incidents/Injuries: None reported.</p>
<p>Description: The recall involves the Safety 1st SmartLight Stair Gate with model number 42111, which is printed on a sticker under the handle panel. The gates, which were manufactured before December 20, 2008, are white metal with a gray handle and a motion sensor nightlight that illuminates on approach. The manufacture date is located below the gate&#8217;s handle as a date wheel imprinted in the plastic; an arrow indicates the month, and the year is stamped inside the circle. The manufacture date is also printed on a sticker inside the battery door.</p>
<p>Sold at: Mass merchandise and juvenile product stores nationwide, including Babies R Us, Toys R Us, Wal-Mart.com and other online retailers from January 2005 through July 2009 for about $60.</p>

<p><strong>Remedy:</strong> Consumers should stop using the recalled gate and contact the company to obtain a free repair kit.</p>

<p><strong>Hazard:</strong> The shoe has decorative wheels that can detach, posing a choking hazard to young children.</p>
<p>Incidents/Injuries: The companies have received two reports of a decorative wheel detaching from the shoe. No injuries have been reported.</p>
<p>Description: This recall involves the Buster Brown &amp; Co. clog children&#8217;s shoes with model or retail numbers listed below. These shoes are a plastic molded EVA &#8216;water&#8217; clog and are designed to resemble a car. The shoes are red, brown, blue, yellow, and pink and most shoes bear the words &#8216;CARS,&#8217; &#8216;Transformers Animated,&#8217; or &#8216;Barbie.&#8217; &#8216;Buster Brown &amp; Co.™&#8217; is printed on the shoe inside at the heel, on a sewn-in tag inside the fleece-lined shoe, and on the original hang tag. The model or retail number can also be found on these tags. The shoes were sold in children sizes 5 to 13 and youth sizes 1-3.</p>

<p>Incidents/Injuries: The firm has received 347 reports of sides of the play yard collapsing unexpectedly, resulting in 21 injuries to young children, including bumps, scrapes, bruises and one concussion.</p>
<p><strong>Description:</strong> This recall involves the Kolcraft Travelin’ Tot play yards, as well as other similar play yards manufactured for Carter’s, Sesame Street, Jeep, Contours, Care Bare and Eric Carle, with model numbers listed below. Some of the units have a bassinet and/or changing table. Some models have a mobile, vibrating unit or a parent organizer. Model numbers are printed on a white sticker on one of the feet of the play yard.</p>

<p><strong>Name of Product:</strong> Simplicity Drop Side Cribs</p>
<p>Units: About 400,000</p>
<p>Importer/Distributor: Simplicity Inc. and SFCA Inc. of Reading, Pa. (The firms appear to no longer conduct day to day operations.)</p>
<p>Hazard: The crib&#8217;s plastic hardware can break or deform, causing the drop side to detach. When the drop side detaches, it creates space between the drop side and the crib mattress. <strong>Infants and toddlers can roll into this space and become entrapped which can lead to suffocation.</strong></p>
<p>Incidents/Injuries: CPSC is aware of one death involving an 8-month-old child from Houston, Texas who became entrapped and suffocated between the drop side and the crib mattress when a plastic connector on the drop side broke. <a href="http://www.cpsc.gov/cpscpub/prerel/prhtml07/07307.html" target="_self">The child&#8217;s death was previously reported</a> by CPSC. CPSC also is aware of an additional 25 incidents involving the drop side detaching from the crib. In six of these incidents, the drop side detached because the plastic flexible tab deformed or broke. In four of the drop side detachment incidents, other plastic parts, including connectors or tracks, deformed or broke. In two of the incidents, two children became entrapped between the drop side and the crib mattress. There were no reported injuries.</p>
<p>Description: This recall involves all drop side cribs with a different or &#8216;newer&#8217; style of plastic hardware from those cribs recalled in September 2007. This newer style of Simplicity hardware can be identified by a flexible plastic tab at the top of the lower tracks.</p>
<p>The recalled model numbers include but may not be limited to: 8050, 8325, 8620, 8745, 8748, 8755, 8756, 8765, 8778, 8810, and 8994, 8995, 8996.

<p>Sold at: Department stores, children&#8217;s stores, and mass merchandisers nationwide from January 2005 through June 2009 for between $150 and $300.</p>

<p><strong>Remedy:</strong> Consumers should immediately stop using the recalled cribs and find an alternative, safe sleeping environment for their baby. Consumers should immediately return the crib to the place of purchase for a refund, replacement or store credit.</p>

<p>Hazard: The play yard&#8217;s rocking bassinet attachment can tilt even when secured by straps in the non-rocking mode or can stay tilted without returning to a level sleeping surface while in the rocking mode, causing an infant to roll to the corner or side of the bassinet. The infant can then become wedged in the corner or pressed against the side or bottom of the bassinet, posing a risk of suffocation or positional asphyxiation.</p>
<p>Incidents/Injuries: The firm and CPSC have received 10 reports of infants rolling to one side, including 6 that had their faces pressed against the side or the bottom of the bassinet. One child reportedly was turning purple and was out of breath when discovered. No injuries have been reported.</p>

<p>Description: This recall involves Eddie Bauer Soothe &amp; Sway portable play yards. The convertible play yard has a bassinet and changing station feature. A mobile with three teddy bears was also sold with the play yard. Models included in the recall are 05046 (all units) and 05044 units manufactured before December 1, 2008. Different models were sold in Canada. Model numbers and manufacture dates are printed on a sticker on one of the support legs underneath the play yard. Manufacture dates are printed in the YY/MM/DD format.</p>
<p>Sold at: Target, Sears, and Burlington Coat Factory stores nationwide and Internet retailers from January 2008 through May 2009 for about $150.</p>

<p>Description: This recall involves blue and red toy maracas with yellow handles. The maracas were used by Tupperware representatives during sales presentations. Cut-outs in the shape of stars, squares and other geometric figures are located inside the maraca.</p>
<p>Sold by: Tupperware&#8217;s Holiday 2007 and Spring 2008 product catalogs from September 2007 through April 2008 for between $5.50 and $11.</p>

<p><strong>Hazard: </strong>The sides of the crib are made of mesh that expands, creating a gap between the side and the crib&#8217;s mattress if it&#8217;s not zipped into place, or a gap between the side and an added mattress. A young child can slip into this gap and become entrapped or suffocate. Also, the crib&#8217;s drop side can fail to fully latch posing a fall hazard to young children.</p>
<p><strong>Incidents/Injuries: </strong>None reported with this crib. However, CPSC is aware of the death of a 5-month-old child in August 2008 involving another company&#8217;s nearly identical crib that was recalled. The child became entrapped between the mattress and the mesh side and suffocated.</p>
<p><strong>Description: </strong>The recall involves the SunKids convertible crib/playpen/bassinet/bed with model number PY256. &#8216;SunKids&#8217; is embroidered on the bottom left of the crib&#8217;s drop side. The convertible cribs have a drop side rail, stationary side rail, canopy assembly, and bassinet. The sides of the convertible crib are fabric and mesh. The mattress support, bassinet, canopy, and bed skirt are covered in fabric. The fabric and the mesh were sold in navy blue, light blue, pink, beige, white, beige checker, and pink heart.</p>
<p><strong>Sold at: </strong>Small juvenile product retailers in N.Y., N. J., and Calif. from January 2007 through October 2008 for about $100.</p>
<p><strong>Manufactured in: </strong>China</p>
<p><strong>Remedy: </strong>Consumers should immediately stop using the recalled convertible cribs and return them to the store where purchased for a full refund.</p>

Each feely box contains ten small stuffed pieces. The piece affected by this recall is the pink and purple butterfly.</p>

<p>Incidents/Injuries: The firm has received 45 reports of the metallic fabric detaching from the toys. No injuries have been reported.</p>

The committee makes recommendations to the Centers for Disease Control and Prevention. Among the latest recommendations: the <strong>Hepatitis-A vaccine</strong> for people who come in contact with international adoptees.</p>
<p>Dr. Cindy Weinbaum, a medical epidemiologist with the CDC, said the recommendations cover adoptions from countries with a high or intermediate infection risk. &#8220;That includes most of the world. All that (it) excludes is North America, Western Europe, Australia, New Zealand and Japan,&#8221; she said. The disease is unfortunately common among children coming from the four top countries of origin for children being adopted by US families. Those are China, Russia, Guatemala and Ethiopia.</p>
<p>Formerly, only parents who were traveling to such countries were recommended for the vaccine. Now, the suggestion includes people such as grandparents and babysitters&#8211;anyone who will have close contact with an international adoptee withing 60 days of his or her arrival in the U.S. The average incubation for the disease is 28 days, but it can be longer, and young children often show only very mild symptoms, thereby increasing the risk to unsuspecting adults. And children adopted from other countries usually have not had a vaccination.</p>
<p>Even though these children only cause 3% (at the highest estimate) of Hep-A infections every year, federal officials state that the results can be tragic&#8211;especially since they are both foreseeable and preventable. The latest of these cases involved a 51-year old woman who became critically ill after exposure to her internationally adopted grandchildren. Weinbaum cited there were about 20 recent cases.</p>
<p>Hepatitis A is a highly contagious liver infection caused by a virus. It is typically spread through feces (in other words, be careful with the diapers!). Symptoms can include fever,<a href="http://www.babiesonline.com/articles/baby/whatisjaundice.asp" target="_self"> jaundice</a>, nausea and vomiting. It is usually more severe in adults, and can be fatal.</p>
<p>The vaccine is given in two doses. The first one should be administered as soon as an adoption is planned. The second would ideally be given two weeks before the child&#8217;s arrival. It is an expensive vaccine, by some standards ($140). But considering that international adoptions often cost between $20,000 and $40,000 (according to Dr. Weinbaum), it&#8217;s not a lot of money in the big scheme of things.</p>
<p>This recommendation follows the news last summer of many <a href="http://blogs.babiesonline.com/health/high-incidence-of-false-negative-tb-tests-in-international-adoptees/" target="_self">false-negative tuberculosis tests</a> in international adoptees. For some reason, the children did not show positive results for three months. Similarly, young children with hepatitis-A don&#8217;t often show signs of jaundice, an indicator for the infection.</p>
