Williams-Sonoma retail stores nationwide and Olivier Napa Valley retail stores located in Truckee and Saint Helena, Calif have received distribution of the product. Though no injuries have been reported as of yet, consumers are being urged to not use Olivier brand Parmesan & Asiago Dip with Garlic & Basil that contain product codes:

Call your local Department of Public Health if you have purchased one of the affected containers.
